Health Insurance Claim Form
CMS-1500 version 02/12
A new Health Insurance Claim Form (CMS-1500 version 02/12) has been designed in preparation for the upcoming ICD10 changes and will accommodate up to 12 diagnostic codes. You can start using the form on January 6, 2014. It will become mandatory on April 1, 2014 so do not order a large quantity of the current form (CMS-1500 version 08/05). Our team is already in the process of getting this new form implemented into the Herfert Software system. Between the dates of January 6, 2014 and March 31, 2014 you will have the option of which form to bill. After April 1, 2014 the new form will be mandatory.
If your support contract with us runs through January of 2014 you will automatically receive these options in a future update.
Herfert Software is ready for the ICD9 to ICD10 changeover which will go into effect October 1st, 2014. We received a special "Thank You" from the BCBSM EDI department for being the 1st software vendor to help them test their system for ICD10.
We will release an update soon that will include an ICD9 to ICD10 translation. The update will allow you to start using ICD10 codes immediately. This will give everyone a full year to get familiar with the ICD10 codes.
Since each diagnosis will have both an ICD9 and ICD10 definition, it will not matter how you enter the diagnosis in the patient's claim file. When a billing is run, our system will convert to the appropriate diagnosis code format based on service dates. Services performed prior to October 1st, 2014 will automatically bill with ICD9 codes and services performed on or after October 1st, 2014 will automatically bill with ICD10 codes.
